Please complete this form, add notes for inactive or PC statuses or special circumstances. You may use this form as many times as is needed.
| Active – The prospect is actively looking and I am working with them or hope to be under broker contract soon. |
| BC – Under Broker Contact |
| PC – Under Property Contract. The closing date is indicated in the comments below. |
| Non-responsive – The prospect has been non-responsive. However the referral is less than 90 days old. |
| Inactive – Inactive/on hold. The prospect has put their search on hold. I have added comments below with details. |
| 86 – This referral is more than 90 days old and is no longer a valid prospect. I understand I am still responsible for the referral fee payment should the lead later close. |
| Paid – Payment has been made on this referral. I’ve given the check number, dollar amount, and when it was paid in the comments section. |
